In a future where humans and Revenants are still struggling to survive, the world is threatened by the return of the Resurgence, a catastrophe capable of unleashing terrifying creatures and causing its collapse. The protagonist, a Revenant Hunter, is saved by Lou MagMell, a Revenant capable of manipulating time. Together, they travel one hundred years into the past to find the heroes whose fates are connected to the seals protecting the present. By altering certain events and uncovering the forgotten truths of that era, they seek a way to prevent the destruction of the world.

Code Vein II continues the Action RPG formula of the first game with a new story centered on the Resurgence, a catastrophe once again threatening humans and Revenants. The player takes the role of a Revenant Hunter accompanied by Lou MagMell, who has the power to travel through time.
The adventure alternates between the present and a period one hundred years in the past. Events discovered in the past help explain the origins of the current situation and allow the player to influence the fate of several Revenants tied to the world's history.
The environments are larger and can be crossed on foot or with the Motorcycle Forma, a summoned bike used to travel more quickly between different regions.
Combat retains a challenging approach built around observing enemies, choosing the right moment to strike and managing abilities. The player can use several weapon categories, including new options such as dual blades and Rune Blades.
Drain attacks allow the player to recover Ichor, the resource required to use many powers. Their behavior depends in part on the equipped Jail, a device worn on the back that modifies certain attributes and available draining actions.
Formae replace part of the traditional Gift system from the first game. They cover different offensive, magical, support and defensive abilities that consume Ichor recovered during combat.
Blood Codes remain at the center of character building. They define different combat styles and can be combined with weapons, Formae and Boosters to create configurations focused on power, mobility, ranged combat or heavy use of abilities.
The new Partner System expands the role of companions. In Summoning mode, the partner fights directly alongside the player. In Assimilation mode, the partner temporarily merges with the Hunter to improve their attributes and abilities.
Partners also provide specific bonuses that evolve with the relationship built with them. They can notably use a Restorative Offering to bring the player back into battle after being defeated, at the cost of becoming temporarily unavailable.
